What is a Generic Drug?

A generic drug is a copy of the brand-name drug with the same dosage, safety, strength, quality, consumption method, performance, and intended use. Before generics become available on the market, the generic company must prove it has the same active ingredients as the brand-name drug and works in the same way and in the same amount of time in the body.

Description

Mesnex (Mesna) is a synthetic medicine that helps protect the body from some of the side effects of certain chemotherapy drugs. It is especially used to prevent a condition called hemorrhagic cystitis, which is bleeding in the bladder caused by the chemotherapy drug ifosfamide. Mesnex works by attaching to harmful substances that ifosfamide creates in the body, making them less dangerous and helping the body get rid of them more safely. Doctors can give Mesnex either through an injection or as a pill.

Mesna is typically administered intravenously (IV) and should be given under the direct supervision of a healthcare provider. The standard dosage of Mesnex can be given through several IV injections or by giving an IV injection followed by oral tablets. The schedule for taking these doses should be repeated on each day that ifosfamide is given. If the dose of ifosfamide changes, the amount of Mesna may need to be changed accordingly.

Contraindications

Do not use Mesnex if you have any of the following conditions:

  • A known allergy to Mesna or any of its ingredients
  • Allergies to thiol compounds, since Mesna is a synthetic sulfhydryl compound

Cautions

  • Before initiating treatment with Mesnex, make sure to disclose the following to your healthcare provider:
    • Any existing health conditions
    • Known allergies
    • A complete list of medications you are currently using
    • Your status regarding pregnancy or breastfeeding
  • Mesnex can trigger severe allergic reactions, such as anaphylaxis. Signs to watch for include fever, hypotension (low blood pressure), rapid heartbeat, respiratory distress, and skin reactions like urticaria (hives). If you notice any of these symptoms, stop taking Mesnex immediately and seek medical help.
  • Mesnex has been linked to serious skin conditions, including those that resemble Stevens-Johnson syndrome or toxic epidermal necrolysis. These can manifest as widespread rashes, blistering, and peeling skin. If you experience any severe skin reactions, discontinue Mesnex and get medical attention right away.
  • The benzyl alcohol in Mesnex injections may be harmful to premature neonates and low-birth weight infants, leading to severe complications like the "gasping syndrome." Use of Mesnex injection should be avoided in these patients. The tablet form of Mesnex does not contain benzyl alcohol and is safer for these groups.
  • Mesnex may interfere with certain lab tests, including those for urinary ketone bodies, causing false positives. It can also affect tests for creatinine phosphokinase (CPK) activity and ascorbic acid, leading to misleading results. Inform your healthcare provider about your Mesnex treatment when undergoing lab tests.

Side Effects

Mesnex is often used alongside the chemotherapy agent ifosfamide, and it can cause various side effects. The most frequent are nausea, vomiting, diarrhea, and constipation, fatigue, fever, loss of appetite, and stomach pain. Other common effects include headaches, hair loss, sleepiness, and changes in blood counts, such as decreased white and red blood cells and platelets, which could lead to increased risk of infections and bleeding.

Mesnex can trigger severe allergic reactions that might occur with your first treatment or after several months. Immediate medical attention is necessary if you notice symptoms like fever, facial swelling, difficulty breathing, severe skin reactions such as rash, hives, redness, blisters, or peeling.
