What is a Generic Drug?

A generic drug is a copy of the brand-name drug with the same dosage, safety, strength, quality, consumption method, performance, and intended use. Before generics become available on the market, the generic company must prove it has the same active ingredients as the brand-name drug and works in the same way and in the same amount of time in the body.

The only differences between generics and their brand-name counterparts is that generics are less expensive and may look slightly different (eg. different shape or color), as trademarks laws prevent a generic from looking exactly like the brand-name drug.

Generics are less expensive because generic manufacturers don't have to invest large sums of money to develop a drug. When the brand-name patent expires, generic companies can manufacture a copy of the brand-name and sell it at a substantial discount.

Description

Nerlynx, also known as neratinib, is a medication used to treat certain types of breast cancer. This drug works as a tyrosine kinase inhibitor, which means it interferes with the action of enzymes that promote cell growth, particularly in cancer cells. It helps block the signals that encourage the cancer to grow, especially in cases where the cancer cells have an excess of a protein called human epidermal growth factor receptor 2 (HER2). It is commonly prescribed to treat HER2-overexpressed or amplified breast cancer and may be used after initial treatments to lower the risk of disease recurrence.

Ingredients

Neratinib is the active ingredient in Nerlynx.

Contraindications

There are no specific contraindications stated on the drug label. However, patients who have demonstrated a severe allergic reaction to any ingredient of Nerlynx should avoid the medication.

Cautions

  • Before initiating treatment with Nerlynx, make sure to disclose the following to your healthcare provider:
    • Any existing health conditions
    • Known allergies
    • A complete list of medications you are currently using
    • Your status regarding pregnancy or breastfeeding
  • Many patients (up to 95%) experience diarrhea, particularly in the first month of treatment. Begin anti-diarrheal medications like loperamide with your first dose, and continue through the first 56 days. If diarrhea becomes severe, involving dehydration or significant discomfort, it’s crucial to notify your doctor who may adjust your treatment or pause Nerlynx temporarily.
  • Liver enzyme increases are a concern with Nerlynx. Regular blood tests are recommended to monitor your liver function before and during treatment. If tests show high levels of liver enzymes, indicating potential liver issues, your doctor might modify your dosage or halt the medication.
  • Nerlynx can harm an unborn baby. Use effective contraception during your treatment and for one month after the final dose. If you suspect you are pregnant, test immediately and inform your doctor if confirmed.

Side Effects

Common Nerlynx side effects include diarrhea, nausea, and stomach pain. Additional common issues include fatigue, vomiting, skin rashes, mouth sores, decreased appetite, and muscle spasms. Some may notice changes in nail color, dry skin, abdominal swelling, nosebleeds, weight loss, and urinary tract infections.

One concern with Nerlynx is its potential to cause liver problems. Monitoring involves blood tests before starting treatment and periodically thereafter, especially during the first three months. If you notice symptoms like fatigue, nausea, pain in the upper right abdomen, fever, rash, itching, or yellowing of the skin or eyes, contact your healthcare provider immediately. These signs could indicate serious liver issues, requiring changes to or discontinuation of treatment.
