What is a Generic Drug?

A generic drug is a copy of the brand-name drug with the same dosage, safety, strength, quality, consumption method, performance, and intended use. Before generics become available on the market, the generic company must prove it has the same active ingredients as the brand-name drug and works in the same way and in the same amount of time in the body.

The only differences between generics and their brand-name counterparts is that generics are less expensive and may look slightly different (eg. different shape or color), as trademarks laws prevent a generic from looking exactly like the brand-name drug.

Description

Procardia XL is a medication used to help with certain heart problems and high blood pressure. It contains an ingredient called Nifedipine, which is part of a group of drugs known as calcium channel blockers. It helps relax the muscles in the heart and blood vessels to improve blood flow and lower blood pressure. Doctors often prescribe Procardia XL to treat high blood pressure (hypertension) and help manage chest pain (angina).

Directions

Procardia XL should be taken exactly as prescribed by your doctor. The extended-release tablets should be swallowed whole, without chewing, crushing, or breaking them. The starting dosage is usually 30 or 60 mg daily and is often determined by individual health factors. Adjustments may be made based on how the patient responds and how blood pressure is monitored. It may be taken with or without food. The titration schedule, typically spanning seven to 14 days, should be followed closely to allow for proper adjustment to each dose.

Contraindications

Avoid taking or using this medication if you have a known hypersensitivity to Nifedipine or have previously experienced allergic reactions to Nifedipine.

Cautions

  • Before starting treatment with Procardia XL, make sure to inform your healthcare provider if you have:
    • Any existing health conditions
    • Any allergies
    • A list of all medications you are currently taking
    • Whether you are pregnant or breastfeeding
  • Nifedipine can cause low blood pressure, especially when starting or increasing the dose. Monitor symptoms like dizziness or fainting, and inform your doctor if they occur. This risk may increase if you're also taking beta blockers.
  • Rarely, Nifedipine can worsen angina (chest pain) or trigger a heart attack, particularly in those with severe coronary artery disease. Report any increase in chest pain immediately.
  • If you're transitioning off beta blockers before starting Nifedipine, do so gradually to avoid withdrawal symptoms like increased chest pain.
  • Nifedipine may cause swelling in the legs or ankles, which is usually due to blood vessel dilation and not heart problems. Notify your doctor if swelling becomes severe or uncomfortable.
  • Rarely, Nifedipine tablets can cause blockage or ulcers in the stomach or intestines. This is more likely if you have existing gastrointestinal conditions. Let your doctor know if you experience abdominal pain or other symptoms.
  • Swallow Procardia XL tablets whole. Do not crush, chew, or break them. The tablet shell may pass in your stool, which is normal.
  • Grapefruit and grapefruit juice can increase Nifedipine levels in your blood, potentially causing side effects. Avoid consuming grapefruit products while taking this medication.
  • Nifedipine may interact with certain drugs, including Digoxin, beta blockers, and medications that affect liver enzymes. Let your doctor know all medications you're taking.
  • Procardia XL should only be used during pregnancy or breastfeeding if absolutely necessary. Discuss potential risks and benefits with your doctor.
  • Older patients may experience higher drug levels due to slower drug clearance. Your doctor may adjust your dosage accordingly.
  • Regular blood pressure checks and, in some cases, lab tests may be needed to monitor your response to treatment and prevent complications.
  • This medication is not recommended for children, as safety and effectiveness have not been established.

Side Effects

Procardia XL may commonly cause peripheral edema (swelling), headache, dizziness, and constipation. Nausea and fatigue were also reported but occurred less frequently. These side effects are generally mild but should be discussed with your healthcare provider if they persist or worsen.

Serious side effects can include gastrointestinal obstruction, which may require medical intervention, and symptoms related to its vasodilator effects, such as palpitations or low blood pressure. Rarely, severe skin reactions like Stevens-Johnson Syndrome or toxic epidermal necrolysis have been reported. If you experience chest pain, significant dizziness, or any unusual symptoms, seek medical attention immediately.
