What is a Generic Drug?

A generic drug is a copy of the brand-name drug with the same dosage, safety, strength, quality, consumption method, performance, and intended use. Before generics become available on the market, the generic company must prove it has the same active ingredients as the brand-name drug and works in the same way and in the same amount of time in the body.

The only differences between generics and their brand-name counterparts is that generics are less expensive and may look slightly different (eg. different shape or color), as trademarks laws prevent a generic from looking exactly like the brand-name drug.

Description

Savella (Milnacipran) is a medication classified as a serotonin-norepinephrine reuptake inhibitor (SNRI). It is approved to treat fibromyalgia, a long-term condition that causes pain all over the body, fatigue, and tender points on the body. Milnacipran works by balancing neurotransmitters in the brain, which can help reduce pain and improve mood and energy levels. While it is not approved for major depressive disorder in the US, other countries might approve it for this purpose.

Directions

Savella should be taken as prescribed by a healthcare provider. It's typically started at a low dosage to reduce the risk of side effects, then gradually increased to an effective dose. The usual starting Savella dosage is 12.5 mg once on the first day, then 12.5 mg twice daily for the next two days, and based on tolerance, increased to 25 mg twice daily on the fourth day. After the initial week, the dose may be increased to 50 mg twice daily. It’s important to swallow the tablets whole and not to crush, chew, or split them. If a dose is missed, take it as soon as you remember unless it's almost time for the next dose. Do not take two doses at once.

Contraindications

Avoid taking or using Savella if any of the following apply:

  • Current or recent (within the last 14 days) use of monoamine oxidase inhibitors (MAOIs) intended to treat psychiatric disorders.
  • Known hypersensitivity to Milnacipran or any of the inactive components found in the medication.

Cautions

  • Before starting treatment with Savella, be sure to inform your healthcare provider if you have:
    • Any existing health conditions
    • Any allergies
    • A list of all medications you are currently taking
    • Whether you are pregnant or breastfeeding
  • Savella may increase the risk of suicidal thoughts or behavior, particularly in young adults under 24 years of age. This risk is highest during the first few months of treatment or after a dose change. Patients and caregivers should watch for any unusual changes in mood or behavior and report these immediately to a healthcare provider.
  • Savella can cause a serious condition called serotonin syndrome, especially when combined with other serotonergic drugs like antidepressants, certain pain medications, or herbal supplements like St. John's Wort. Symptoms may include agitation, confusion, rapid heart rate, sweating, or muscle stiffness. If these symptoms appear, stop using Savella and seek medical help immediately.
  • Savella can increase blood pressure and heart rate, which may lead to complications in patients with heart conditions or high blood pressure. Blood pressure and heart rate should be monitored regularly, and patients experiencing sustained increases may need a dose adjustment or to stop treatment.
  • Savella may cause mild increases in liver enzyme levels, and rare cases of severe liver injury have been reported. Patients with liver disease or significant alcohol use should avoid Savella, and any signs of liver problems, like yellowing of the skin or eyes, should warrant immediate medical attention.
  • Stopping Savella suddenly can lead to withdrawal symptoms like dizziness, irritability, or sleep disturbances. Always taper off Savella under the guidance of a healthcare provider to reduce the risk of these effects.

Side Effects

The most common side effects of Savella include nausea, dizziness, and insomnia. Other frequently reported reactions are increased sweating (hyperhidrosis), constipation, and increased heart rate. Hot flashes and dry mouth were also common among patients during clinical trials. These side effects were generally mild and often improved over time.

Savella may cause more serious side effects, such as elevated blood pressure, palpitations, or serotonin syndrome, particularly when combined with certain other medications. Rare cases of severe skin reactions like Stevens-Johnson syndrome and severe liver issues such as hepatitis have been reported. In male patients, genitourinary side effects such as difficulty urinating or decreased libido were noted. If you experience symptoms such as severe rash, trouble breathing, or unusual changes in mood or behavior, contact a healthcare provider immediately.
