What is a Generic Drug?

A generic drug is a copy of the brand-name drug with the same dosage, safety, strength, quality, consumption method, performance, and intended use. Before generics become available on the market, the generic company must prove it has the same active ingredients as the brand-name drug and works in the same way and in the same amount of time in the body.

The only differences between generics and their brand-name counterparts is that generics are less expensive and may look slightly different (eg. different shape or color), as trademarks laws prevent a generic from looking exactly like the brand-name drug.

Description

Surmontil (trimipramine) is a medication prescribed to alleviate the symptoms of depression.

The generic name, trimipramine, comes in 25 mg, 50 mg, and 100 mg capsules for oral administration.

Directions

Surmontil (trimipramine) should be taken as your doctor has instructed. Your doctor will determine an appropriate Surmontil (trimipramine) dosage and treatment regimen for you.

Store the medication in its tightly closed container and keep between 20°C and 25°C (68°F and 77°F).

Ingredients

Trimipramine is the active ingredient in Surmontil.

Lactose monohydrate and magnesium stearate are inactive ingredients.

D&C Yellow 10 (25 mg), FD&C Blue 1 (25 mg, 50 mg and 100 mg), FD&C Yellow 6 (25 mg, 50 mg), D&C Red 28 (50 mg), FD&C Red 40 (50 mg), gelatin, and titanium dioxide are contained in the capsule shell. The black ink that is imprinted on the capsules contains shellac glaze, iron oxide black, propylene glycol, and ammonium hydroxide.

Contraindications

Do not take Surmontil (trimipramine) if you are allergic to any tricyclic antidepressants or if you are recovering from a heart attack.

Do not take monoamine oxidase inhibitors (MAOIs) for psychiatric disorders with Surmontil (trimipramine) or within 14 days of stopping Surmontil (trimipramine). In addition, do not take Surmontil (trimipramine) within 14 days of stopping an MAOI for psychiatric disorders.

Surmontil (trimipramine) should not be started in patients who are being treated with linezolid or intravenous methylene blue.

Cautions

  • Surmontil (trimipramine) contains the following boxed warning:
    • The risk of suicidal thinking and behavior in children, adolescents, and young adults was increased with antidepressants compared to placebo according to short-term studies of major depressive disorder and other psychiatric disorders. The risks and benefits must be evaluated. The increased risk of suicidal thinking and behavior with antidepressants compared to placebo in adults older than 24 years of age was not shown in short-term studies, and the risk was reduced in adults 65 years of age and older. The risk of suicide is increased with depression and other psychiatric disorders themselves. Appropriate monitoring and close observation for clinical worsening, suicidal thinking and behavior, or unusual behavioral changes should occur in patients of all ages who begin treatment with antidepressants. Families and caregivers should closely observe and communicate with the prescriber.
  • Before you begin treatment with Surmontil (trimipramine), inform your doctor of:
    • All the prescription and nonprescription medications you take
    • Your allergies
    • Your current health conditions
    • Your past medical history
    • Your pregnancy or breastfeeding status
  • Surmontil (trimipramine) contains the following warnings and precautions:
    • Clinical worsening and risk of suicide
    • Patients with symptoms of depression should be screened for bipolar disorder risk before they begin taking an antidepressant.
    • Serotonin syndrome, especially when taken with other medications that can increase serotonin levels (i.e., triptans, tricyclic antidepressants, fentanyl, lithium, tramadol, tryptophan, buspirone, and St. John’s wort). Symptoms may include but are not limited to changes in mental status, rapid heart rate, fluctuating blood pressure, dizziness, sweating, flushing, hyperthermia, tremor, rigidity, coordination issues, seizures, and gastrointestinal symptoms.
    • Angle-closure glaucoma
    • Caution should be used in patients:
      • With cardiovascular disease
      • Who have a history of urinary retention
      • With hyperthyroidism or taking thyroid medication
      • Who have a history of seizure disorder
      • Receiving guanethidine or similar agents
    • Do not operate machinery, drive, or participate in other hazardous activities until you know how the medication affects you.

Side Effects

Surmontil (trimipramine) side effects may include but are not limited to nausea, vomiting, diarrhea, stomach cramps, drowsiness, weakness/tiredness, restlessness, anxiety, confusion, dizziness, headache, nightmares, appetite or weight changes, dry mouth, constipation, trouble urinating, frequent urination, changes in sex drive or ability, excessive sweating, ringing in the ears, rash, enlargement of the breasts, abnormal blood cell counts, and pain, burning, or tingling in the hands and feet.

Immediately contact your doctor if you develop bothersome or persistent side effects while taking Surmontil (trimipramine).
