What is a Generic Drug?

A generic drug is a copy of the brand-name drug with the same dosage, safety, strength, quality, consumption method, performance, and intended use. Before generics become available on the market, the generic company must prove it has the same active ingredients as the brand-name drug and works in the same way and in the same amount of time in the body.

Description

Trikafta (elexacaftor/tezacaftor/ivacaftor and ivacaftor) contains 3 medications for the treatment of cystic fibrosis in patients who are 2 years of age and older with one or more F508del mutations or another mutation in the CFTR gene that responds to treatment.

Trikafta is available in oral tablets:

  • Fixed-dose combination containing elexacaftor 50 mg, tezacaftor 25 mg and
    ivacaftor 37.5 mg co-packaged with ivacaftor 75 mg
  • Fixed-dose combination containing elexacaftor 100 mg, tezacaftor 50 mg,
    and ivacaftor 75 mg co-packaged with ivacaftor 150 mg

Trikafta is also available in oral granules:

  • Unit-dose packets of elexacaftor 100 mg, tezacaftor 50 mg and ivacaftor
    75 mg co-packaged with unit-dose packets of ivacaftor 75 mg
  • Unit-dose packets of elexacaftor 80 mg, tezacaftor 40 mg and ivacaftor
    60 mg co-packaged with unit-dose packets of ivacaftor 59.5 mg

Directions

Trikafta should be taken as directed by your doctor. The instructions for use that accompany your prescription should be thoroughly read. Any questions or concerns may be referred to your doctor or pharmacist.

Your doctor will instruct you on how much Trikafta you should take and for how long, depending on your age, weight, and medical conditions. Trikafta should be taken with fatty food to aid its absorption into the body.

Ingredients

The active ingredients in Trikafta include elexacaftor/tezacaftor/ivacaftor.

Croscarmellose sodium, hypromellose, hypromellose acetate succinate, magnesium stearate, microcrystalline cellulose, sodium lauryl sulfate, hydroxypropyl cellulose, hypromellose, iron oxide red, iron oxide yellow, talc, and titanium dioxide are the inactive ingredients in the elexacaftor, tezacaftor and ivacaftor fixed-dose combination tablets.

Colloidal silicon dioxide, croscarmellose sodium, hypromellose acetate succinate, lactose monohydrate, magnesium stearate, microcrystalline cellulose and sodium lauryl sulfate, carnauba wax, FD&C Blue #2, PEG 3350, polyvinyl alcohol, talc, titanium dioxide, ammonium hydroxide, iron oxide black, propylene glycol, and shellac are the inactive ingredients in the ivacaftor tablets.

Colloidal silicon dioxide, croscarmellose sodium, hypromellose, hypromellose acetate succinate, lactose monohydrate, magnesium stearate, mannitol, sodium lauryl sulfate, and sucralose are the inactive ingredients in the elexacaftor, tezacaftor and ivacaftor fixed-dose combination oral granules.

Colloidal silicon dioxide, croscarmellose sodium, hypromellose acetate succinate, lactose monohydrate, magnesium stearate, mannitol, sodium lauryl sulfate, and sucralose are the inactive ingredients in the ivacaftor granules.

Contraindications

The most recent Prescribing Information does not list any contraindications to Trikafta. However, consult your healthcare provider about your treatment options.

Cautions

  • Before you start taking Trikafta, inform your doctor of:
    • All the prescription and over-the-counter medications you take
    • Your allergies
    • Your current health problems
    • Your past medical history
    • Your pregnancy or breastfeeding status
  • The following conditions may result from Trikafta use:
    • Increased liver enzymes and liver problems. Trikafta is not recommended in patients with advanced liver disease unless the benefits outweigh the risks.
    • Allergic reactions such as angioedema (swelling) and anaphylaxis.
    • Cataracts
  • Taking Trikafta with CYP3A inducers (i.e., rifampin St. John’s wort) is not recommended, as taking them together can affect how Trikafta works in the body. Ask your doctor if you do not know whether you take CYP-3A inducers.

Side Effects

Trikafta side effects include but are not limited to headache, upper respiratory tract infection, abdominal pain, diarrhea, rash, increased liver enzymes, nasal congestion, blood creatine phosphokinase increased, runny nose, rhinitis, influenza, sinusitis, and increased blood bilirubin. Immediately inform your doctor of any bothersome or persistent side effects.
