What is a Generic Drug?

A generic drug is a copy of the brand-name drug with the same dosage, safety, strength, quality, consumption method, performance, and intended use. Before generics become available on the market, the generic company must prove it has the same active ingredients as the brand-name drug and works in the same way and in the same amount of time in the body.

The only differences between generics and their brand-name counterparts is that generics are less expensive and may look slightly different (eg. different shape or color), as trademarks laws prevent a generic from looking exactly like the brand-name drug.

Generics are less expensive because generic manufacturers don't have to invest large sums of money to develop a drug. When the brand-name patent expires, generic companies can manufacture a copy of the brand-name and sell it at a substantial discount.

Description

Zelboraf (Vemurafenib) is a medication that helps fight cancer. It works by blocking a specific enzyme in the body known as B-Raf, which is important for cancer cell growth. By stopping this enzyme, Zelboraf helps to slow down or even kill cancer cells. This drug is mainly used to treat advanced melanoma, a type of skin cancer, especially in patients whose cancer has a specific change in their genes called the BRAF V600E mutation. Zelboraf comes in the form of tablets that you take by mouth. It can also be used to treat Erdheim-Chester Disease, which is a rare type of blood cancer.

Directions

Zelboraf is typically taken twice a day, around 12 hours apart. It's important to swallow the tablet whole with a glass of water, and you may take it with or without food. If you miss a dose and it's less than four hours until the next one, skip the missed dose. The standard dosage often starts at 960 mg twice a day, but it may be adjusted based on individual response and tolerability. Follow the doctor's instructions carefully and do not change the dosage without consulting them.

Contraindications

Zelboraf's drug label does not specify any particular contraindications. However, it might not be the right choice for everyone. If you know you are allergic to Vemurafenib or any of its ingredients, you should avoid taking this medication.

Cautions

  • Before starting treatment with Zelboraf, make sure to disclose the following to your healthcare provider:
    • Any existing health conditions
    • Known allergies
    • A complete list of medications you are currently using
    • Your status regarding pregnancy or breastfeeding
  • Zelboraf can increase the chances of developing skin cancers like squamous cell carcinoma and melanoma. Regular skin checks by your doctor are recommended every two months during treatment and for six months after stopping Zelboraf.
  • Non-cutaneous squamous cell carcinomas, especially in the head and neck, can occur. Your doctor will monitor for signs of new cancers.
  • Zelboraf can promote tumor growth in certain types of melanoma without the BRAF V600E mutation. Make sure your tumor is tested for this mutation before starting Zelboraf.
  • Severe allergic reactions, including rash, swelling, and low blood pressure, can happen. If you experience these, stop using Zelboraf immediately and seek emergency help.
  • Zelboraf can cause dangerous skin conditions like Stevens-Johnson syndrome. If severe skin reactions occur, the medication must be permanently stopped.
  • Zelboraf can lead to heart rhythm issues (QT prolongation), which can cause serious arrhythmias. Regular monitoring of your heart and electrolyte levels is necessary.
  • Zelboraf can cause liver problems, including liver failure. Your liver function will need to be checked regularly with blood tests.
  • Zelboraf can make you more sensitive to the sun, leading to severe sunburn. Avoid direct sunlight, wear protective clothing, and use sunscreen with SPF 30 or higher.
  • Uveitis (eye inflammation), blurry vision, and sensitivity to light may occur. If you notice vision changes, consult your doctor.
  • Zelboraf can cause harm to an unborn baby. If you're pregnant or could become pregnant, use effective birth control during and for two weeks after treatment.
  • Zelboraf can increase the risk of severe reactions to radiation therapy, including fatal cases. Your doctor will closely monitor you if you’re receiving radiation therapy.
  • Zelboraf can cause kidney problems. Your kidney function will be monitored during treatment.
  • Some people may develop Dupuytren’s contracture or plantar fascial fibromatosis, which can cause hand or foot deformities. Let your doctor know if you experience pain or difficulty moving your hands or feet.

Side Effects

Zelboraf commonly causes side effects such as joint pain (arthralgia), rashes, hair loss (alopecia), and fatigue. Other frequent reactions include increased sensitivity to sunlight, nausea, itching (pruritus), and small skin growths known as papillomas. These side effects are often mild and manageable but should be discussed with your healthcare provider if they worsen.

In patients with Erdheim-Chester Disease, common side effects include joint pain, maculo-papular rashes, hair loss, and fatigue. Additionally, prolonged QT intervals, which affect heart rhythm, and skin papillomas may occur. If you experience heart-related symptoms, severe skin reactions, or persistent fatigue, contact your doctor for further evaluation.
